II.1.5) Short Description of the contract or purchase(s) Telford & Wrekin Council wish to invite tenders from potential providers for the provision of a minimum of ten intermediate care beds for older people within a residential setting in the Borough of Telford & Wrekin. The successful provider will be awarded a 5 year contract with an option to extend for a further 3 years at the discretion of the Council. Ideally the beds will need to be in a single unit, but alternative options will be considered. In addition the Council may purchase additional beds to support the strategic approach to re-ablement and enablement within the Borough.

II.2) QUANTITY OR SCOPE OF THE CONTRACT
II.2.1) Total quantity or scope (including all lots and options, if applicable) Due to the impending expiry of an existing contract, Telford & Wrekin Council wish to invite tenders from potential providers for the provision of a minimum of ten intermediate care beds for older people within a residential setting in the Borough of Telford & Wrekin. Ideally the beds will need to be in a single unit, but alternative options will be considered by the Council. In addition the Council may purchase additional beds to support the strategic approach to re-ablement and enablement within the Borough. The existing contract will expire 31 March 2011 and the revised contract will come into effect from 1st April 2011. The revised contract will be for a period of 5 years, with the option to extend up to a further 3 years. Potential providers must be registered with the Care Quality Commission as providers of residential care services by the 1st April 2011 and preferably should have previous experience of delivering residential and / or nursing care services. Intermediate Care has an important function in meeting the health and social care needs of individuals to prevent unnecessary admission to hospital, expedite hospital discharge and avoid premature admission into care homes. Older people are particularly vulnerable at transitions points so services are needed that work together ensuring that appropriate care is provided in the right place at the right time. Intermediate Care is based on active enablement and encompasses a wider preventative role aiming to promote confidence building and social inclusion thus avoiding the need for more institutional care.
