II.1.5) Short Description of the contract or purchase(s) Telford & Wrekin Council wish to invite tenders from potential providers who are interested in delivering care and support services to people living within a newly developed Extra Care Scheme which is due to open in the Borough in August 2011. The contract available will be for the provision of personal care and housing related support to tenants living within the Scheme. The Council will guarantee a minimum pre-purchase commitment of hours of care and support into the service. The contract will be for a five year period, with the option to extend up to a further 5 years subject to satisfactory performance. II.2) QUANTITY OR SCOPE OF THE CONTRACT
II.2.1) Total quantity or scope (including all lots and options, if applicable) Telford & Wrekin Council wish to invite tenders from potential providers who are interested in delivering care and support services to people living within the new Bournville House Extra Care Scheme at Lightmoor Village, Telford. Part of Telford & Wrekin Council's Housing, Care and Support Strategy is to develop an additional 500 extra care units of accommodation by 2021. As such, a new Extra Care Housing Scheme (Bournville House) is due to open in the Borough in August 2011. Bournville House is being built by the Bournville Village Trust who will be the Registered Social Landlord for the scheme when it opens. Bournville House will comprise of 59 self-contained 2 bedroom apartments. The service is due to start early August 2011 subject to the build programme completion. The contract available will be for the provision of personal care and housing related support to tenants living in the 59 units at Bournville House. The model of service delivery is based on a balanced community approach of a third low, a third medium and a third high dependency. The Council will guarantee a minimum pre-purchase commitment of hours of care and support into the service. The provision of services will be primarily for older people, including older people with mild to moderate mental health needs. Services will be delivered using a person centred approach to help people to remain as independent as possible in their local communities and promoting their quality of life and wellbeing. The service model is one of core Care and Support with the Council guaranteeing to purchase a minimum amount of hours. There is also the potential option to develop an outreach service into the surrounding community. Potential providers must be committed to the principles within the Governments White Paper - 'our health, our care, our say' and to the vision set out in the 'Putting People First' concordat. To this end, potential providers are expected to support the Council in transforming their social care services with an emphasis on reablement, rehabilitation and service personalisation. Any potential provider must be registered with the Care Quality Commission. Applications are invited preferably from established organisations with previous experience of delivering services within Extra Care Schemes. Consideration may also be given to applicants with a proven track record of delivering person centred outcome focused care in other registered settings.
